Consent to the processing of health data

This document is a form of consent to the processing of a special category of personal data — information about the state of health — in accordance with Articles 8 and 13 of Law of the RK “On personal data and its protection” No. 94-V of 21.05.2013.

1. Who processes the data

2. What data is involved

When using the pharmacy chain's services, the following may be processed:

• information about the prescription medicines purchased and their intended use;
• information about chronic conditions provided when issuing a loyalty card or in your profile;
• information about reactions to medicines, if you have reported them;
• information about allergies and contraindications (when filling in your profile).

3. Purposes of processing

• Pharmaceutical advice and warnings about incompatible medicines.
• Personalised recommendations on equivalents and dosages.
• Compliance with the legislation on the circulation of medicines.
• Keeping an order history for convenient repeat purchases.

4. Legal basis and voluntary nature

Health data is processed solely on the basis of the voluntary consent of the data subject. Providing such data is voluntary. Refusing to provide it does not restrict your right to buy goods at the chain's pharmacies — but personalised recommendations will not be available.

5. Transfer to third parties

Health data is not transferred to third parties for commercial purposes. Transfer is possible only:

• at the lawful request of state bodies of the RK (for example, the Ministry of Health);
• to medical staff in an emergency (with your consent).

6. Retention period

The data is stored for as long as the loyalty card / personal account is valid and is destroyed no later than 30 days after the account is deleted or consent is withdrawn.

7. Your rights

You have the right to:

• withdraw your consent at any time by sending a request to info@gippokrat.kz;
• demand that the data be corrected, blocked or destroyed;
• obtain information about to whom and when your data was transferred.

Withdrawing consent does not affect the lawfulness of processing carried out before the withdrawal.
